Completed Two Weeks Prior to the Activity

  • Send a copy to Mr. Argo, tbargo@alvaschools.net
  • Submit your form to your site principal, or if you are a sport to the Athletic Director.
  • The site Principal/Athletic Director will approve the request and send to the Transportation Director.
  • The Transportation Director will email the Princpal/Athletic Director the vehicle that will be used for the activity.
